I am pleased to share the third case in our biweekly clinical series. We continue our focus on the treatment of periodontitis, with particular emphasis on periodontal regeneration.

Periodontal regeneration is defined as “the reproduction of lost periodontal tissues in a manner similar to their original form, including the formation of new bone, cementum, and a functionally oriented periodontal ligament at a site previously deprived of its original attachment apparatus” (Glossary of Periodontal Terms, 2021). Clinically and radiographically, regenerative therapy can result in reduced probing depths, gain in clinical attachment level, radiographic bone fill, decreased tooth mobility, and improved long-term tooth prognosis.

Periodontal regeneration is indicated in cases with residual probing depths following scaling and root planing and the presence of either a localized interproximal infrabony defect or a Class II furcation involvement. Among the various approaches available, the most predictable techniques involve the use of bone grafts in combination with collagen membranes or biological agents.

This case illustrates the treatment of a Class II furcation defect using periodontal regenerative therapy. Following thorough debridement and subgingival calculus removal, the defect was treated using bone particles and a collagen membrane.

The treated site demonstrated significant radiographic bone gain and probing depth reduction. The clinical response was excellent, with an outcome that was very satisfying for both the patient and the clinical team. The prognosis of the tooth improved from questionable to favorable.

This educational series highlights real-life clinical scenarios to raise awareness of periodontal disease and demonstrate the effectiveness of contemporary treatment options. We hope this initiative inspires confidence in the management of compromised dentition. As always, I am here to support you and your patients.
